(uniscribe_shape): Increase items buffer size.

Give up if glyph indices not supported.  Use uniscribe obtained
ABC widths for individual metrics.  Map glyph clusters back to
characters using fClusterStart flag.  Return number of glyphs
produced, not chars processed.
