(sgml-lexical-context): Return (text . START)

instead of nil when point is outside of any tag.
(sgml-beginning-of-tag): Adjust to the change.
(sgml-calculate-indent): Use the new info returned by
sgml-lexical-context.  Try to handle unclosed tags.
